There's an additional layer with fluoxetine specifically: it's not just a CYP2D6 substrate — it's also a CYP2D6 inhibitor. How to navigate this practically

  1. Tell your prescriber before starting CBD. I did. Mention which antidepressant, at what dose, and for how long you've been on it. If you've had pharmacogenomic testing, share the CYP2D6 result, it's directly relevant to your baseline risk before CBD enters the picture.
  2. If you're on fluoxetine specifically, understand the double-inhibition dynamic. Prozac is both a CYP2D6 substrate and a CYP2D6 inhibitor. Adding CBD means two inhibitors on the same enzyme. This also affects other CYP2D6-cleared medications you may be taking, metoprolol and certain opioid pain medications are examples. This is worth an explicit conversation with your prescriber, not just a checkbox.
  3. Know what's normal for you so you can spot a shift. Over-exposure signs, unusual nausea, agitation, tremor, insomnia, or amplified sexual side effects, overlap with what higher antidepressant doses feel like. These are generally gradual, not sudden. If severe symptoms appear (high fever, rapid heart rate, muscle rigidity, confusion), that's a different category, call emergency services immediately.
